Wider turn would have helped some there too.

I drove fire engines for most of my career. I had the mind set that the public is doing what they do everyday while driving. Then I show up lights, siren big red shiney fire engine causing all kinds of disturbance. So really I'm the issue so I gave everyone the time to figure out what to do. I'm not doing my job if I don't get to and from an emergency safely. Crew and engine intact.

Seen this video in my EVOC class in June and always told “never speed and slow down especially on turns to avoid a collision”

In my state if you bust a red light and there is an accident you are at fault. Slow down. I have actually been in the right seat and had a car go through an intersection with us setting at the intersection with all the lights and sirens in full operation.

No not an accident... Just pure stupidity on the drivers part... What the duce was he doing? Auditioning for need for speed 12 in drifting an articulated ladder truck at that speed through a 90 degree turn... At best he should be no faster than 15 MPH through that turn. The same as if it were an 18-wheeler.

Check the intersection more often, you check every intersection. As a Driver as I approach an intersection, I would take my right foot off the gas and place it on the brake pedal. That became an SOP in our Dept. You were all at fault, pray to God Captain that YOUR driver didn't kill someone YOU should have told him to slow his ass down, Tiller Driver you also. What you damn near killed 3 people and destroyed a what 2 million Dollar Tiller Rig. its junk,, tunnel vision 100% I drove for almost 12 years, I never put one scratch on my pumper. Learn from your mistakes or die from them.

Moral of the story, slow the fuck down when cornering, cant fight the fire if you don't get there.

The basic rules of EVOC training were just ignored here. They were driving way, WAY too fast for the turn. They completely blew a red light too. As first responders we're trained that we can't help anyone if we can't get there at all. I will say that it takes discipline to keep the impulse of driving fast when the lights and siren kick on at bay. You also have to ignore response times and the urge to meet them. Ultimately response time responsibility falls on dispatch and leadership and I refuse to push the limit and endanger myself, my partner and the public to meet response times. Your adrenaline also starts and you can develop tunnel vision to a certain extent. But that's where disciplined training takes over. I've been lucky that I never got in a wreck in the thousands of responses I've been on.

Simple: Took the turn too fast. It happens.

after watching this video , I feel that the narrative is to make a Hero out of Zero. many mistakes and missteps were made by many featured in the video but yet the somber soundtrack plays and the commentary of the sorry mistakes were over played.

It took a lot of courage for this department to post this video, and I commend them. As a former Firefighter/Engineer, I'm shocked that individuals did not have their seatbelts fastened. If I were to design a fire truck today, I would demand that sensors be installed in all stations with interlocks, preventing the transmission from being placed into 'drive' unless the seatbelts are locked in position. Additionally, that tiller cab should not have crushed so easily. I would change the specs to reinforce the Tiller frame and airbags, much in the same way as some cabs. Finally, I believe some measures could lower the center of gravity of the trailing chassis. While it's abundantly clear what caused this accident, my comments are centric to design versus training.
